Sinking of bone flap-looking beyond cosmesis and costs

چکیده
Dear Editor, Decompressive craniectomy is a common procedure performed for raised intracranial hypertension with obligatory subsequent bone flap replacement or cranioplasty at varying intervals, for various side effects of the lack of bone flap, well described in the literature. However, the complications during and following the replacement of bone flap did not strike the limelight, which may require another surgical intervention.[1‐3] Sinking of entire bone flap following the cranioplasty procedure is much more than cosmesis for the antecedent neurological deficits it produces; hence righteously qualifies to be termed as “sunken bone flap syndrome”.

Reperfusion Injury after Autologous Cranioplasty in a Patient with Sinking Skin Flap Syndrome
The sinking skin flap syndrome is a rare complication after a large craniectomy. It consists of a sunken skin above the bone defect with neurological symptoms such as severe headache, mental changes, focal deficits, or seizures. In patient with sinking skin flap syndrome, cerebral blood flow and cerebral metabolism are decreased by sinking skin flap syndrome, and it may cause the deterioration ...
متن کاملCase report of MR perfusion imaging in Sinking Skin Flap Syndrome: growing evidence for hemodynamic impairment
BACKGROUND The syndrome of the sinking skin flap (SSSF) with delayed sensorimotor deficits after craniectomy is not well known and often neglected. Among various postulated causes, there is evidence that disturbed brain perfusion may be related to the observed symptoms, and that cranioplasty reliably alleviates these symptoms. We report a case of sinking skin flap syndrome (SSFS) with recovery ...
متن کاملCT perfusion imaging in the syndrome of the sinking skin flap before and after cranioplasty.
The syndrome of the sinking skin flap (SSSF) has been described as one of the causes of neurological deficits after decompressive craniectomy We report a case of a 57-year-old woman with SSSF. Two years earlier, this patient, with no neurological deficits, underwent removal of the bone flap during treatment of an epidural abscess due to wound infection after a clipping operation for a ruptured ...

Introduction: This study was designed to evaluate the usefulness of mastoid cavity obliteration with combined bone pâté and Palva flap in the prevention of problematic mastoid cavities after canal wall down mastoidectomy. Materials and Methods: In a prospective longitudinal study with a mean follow-up of 28 months conducted between 2008–2012, a series of 56 ears in 48 patients with chronic ot...

The biocompatibility and osseointegrative qualities of the element titanium have allowed its extensive use medical science especially Orthopedics and Neurosurgery, effectively replacing its predecessor, steel. Historically, nonabsorbable sutures were used to anchor the bone flap following a craniotomy but have been associated with functional and cosmetic defects as well as nonunion and failure ...
